In 1915, at Boston, MA, Howard married Frances Doane Barber who was born on October 10, 1893, a daughter of Francis and Lucy Fullerton Barber<ref name="Bernehistory"/>. They had one daughter:

DR. HOWARD SETTLE DIED AT BOSTON ON MONDAY Succumbed to Peritonitis Following Operation for Appendicitis -- Was Only Son of Mr. and Mrs. Theodore Settle of Berne. 
On Monday, Dec. 20, the sad news came to Berne of the death, at Boston, Mass., of Dr. Howard E. Settle. On Dec. 11 Dr. Settle underwent an operation for acute appendicitis, and peritonitis developed, with fatal results. Dr. Settle was born in Berne, the only child of Mr. and Mrs. Theodore Settle, and was in his twenty-ninth year. His attractive personality won for him friends whom he held by the admirable qualities of mind and character which he possessed. Dr. Settle was graduated from Harvard University, where he also completed the medical course. Soon after leaving the University he married Miss Frances Barber of Montpelier, Vt. During the late war, while Dr. Settle was in the service as examining surgeon on a training ship, Mrs. Settle, with their infant daughter, was in the family home in Berne; and those who came to know her charming and winsome nature sympathize with her deeply in this tragic ending of a most happy married life. The utter desolation that this loss brings to his honored parents can be realized only by those who have experienced like sorrow. Dr. Settle was a man of Christian faith and principle, a member of the church from early youth, and heartily loyal to the truths which he held sacred. To the many who mourn, the end of this noble life seems most untimely; but we realize that God's ways are not as our ways, and that in infinite wisdom He accomplishes His eternal purposes. The funeral service was held in Boston on Wednesday; and the interment was at Berne, in the family plot in the Beaverdam cemetery.
